I have successfully used the freeware help tool Oasis which is available from
http://fire.adept.co.za/~oasisdev/
Its description (from the website):
Oasis SE (Standard Edition) is a Freeware help authoring tool for
Windows 95/ NT4.x. It provides the user with an integrated interface
for writing Windows 95/NT4 Help Files. This product is compatible
with Microsoft Windows Help version 4.0.
